Additional Enrollment Requirements

1. The applicant must have satisfactory physical and mental health. A physical examination and health history is required after admission. It must be submitted by August 1. All immunizations and TB test must be current.
2. A security screening will be completed prior to any clinical experience. Criminal conviction may jeopardize the ability to continue in the course and/or licensure.
3. The applicant shall provide evidence of current CPR certification at the professional level by August 1.
4. Applicants to the associate degree level must be eligible to enroll in or have completed English Composition I and Intermediate Algebra/Survey of Math at the beginning of the academic year.
5. Courses required for graduation, if taken online, must be completed by January 10, prior to the beginning of NALS V.
