Babies Sleep on Their Backs Here!

SAFE SLEEP POLICY

This facility has a Safe Sleep Policy.  All infants will be placed on their backs to sleep to reduce the risks of SIDS.  Though SIDS is unexpected, certain precautions can be taken to help reduce the risks to your child.  In addition, stuffed animals, fluffy blankets, and other soft items will be removed from the child’s sleeping area during sleep.  A clean, sturdy pacifier may be used by the child during sleep, as it has been found that this can additionally reduce the risks of SIDS.  This policy is for the protection of the child.

If you prefer your child sleep on his/her stomach, you must supply a signed and notarized waiver of responsibility that states this is your preference.  In addition, you must also provide a signed and dated Doctor’s note describing why this child should be placed on their stomach to sleep.  Without such documents on file, the child will be placed on their back for sleep only!

Thank you for your understanding about this difficult issue.  If you would like more information about SIDS, I would be glad to supply you with as much information as I can.  You can also refer to the SIDS Network of Kansas at www.angelfire.org.

I have read the above policy and understand that it is for the safety & protection of my child.  I agree to the above terms and agree that without a notarized waiver and Doctors note, my child will be placed on their back to sleep.
